Introducing our Patient Care Navigator

Introducing our Patient Care Navigator

Linc Telecom introduces its new telephone-based self-service appointment system that syncs with the practice's existing telephone system.

Reduce patient call-backs and delayed booking processes. Patient Care Navigator works with the practice rule book, ensuring correct appointments are booked the first time around including:

  • Vaccinations
  • Nurse Appointments
  • Third party Pharmacy Referrals

Patient Care Navigator powered by Linc Group enables GP practices to offer patients the ability to book, check, change and cancel appointments 24/7 while also signposting to external services if required. The software can work alongside a new cloud-based telephone service or added to an existing onsite or cloud based telephone service, as well as all primary clinical databases; EMIS, SystmOne and Vision. This means patient access can be improved upon seamlessly while meeting the GP contract requirements.

Benefits

Utilising Patient Care Navigator has numerous benefits to the surgery or group of practices;

  • Ensure patients reach the correct point of care on first contact with NHS practices. The system can do so by booking the first available appointment or signposting to relevant external services, on a self-referral basis, such as pharmacies
  • Reduce patient call-backs and delayed booking processes. Patient Care Navigator works with your rule book, ensuring the correct appointment is booked the first time around. Not limited to GP appointments alone, the system covers a wide variety of appointment
  • In addition to its self-referral capabilities, Patient Care Navigator provides a range of other features designed to enhance the patient experience. Its user-friendly interface, referral tracking system, and access to additional healthcare services such as mental health support make it a comprehensive solution for practices of all sizes.
